Effective June 1, 2015, Rule 66 of the Rules of Superintendence for the Courts of Ohio was amended to help better protect the wards in guardianships. These changes were adopted and included in our local rules.

Effective January 1, 2015 the Supreme Court has revised/created the following forms:
14.0 Application to Approve Settlement and Distribution of Wrongful Death and Survival Claims
22.5 Application to Settle a Claim of an Adult Ward
22.6 Entry Approving Settlement of Claim of an Adult Ward
22.7 Report of Distribution and Entry

Effective June 1, 2014 the Supreme Court has revised/created the following forms:
7.0 Certificate of Notice to Administrator of Medicaid Estate Recovery Program

The Probate Court has created check lists for the following case types:

Effective March 1, 2014 a Fiduciary's Acceptance will be required for all new Full Administrations.

As of January 1, 2013, Application for Appointment of Guardian of Alleged Incompetent has been revised. A Supplemental Application for Emergency Guardian is now required to be filed along with the Application for Appointment of Guardian of Alleged Incompetent.

TAX ALERT: Ohio Estate Tax returns are no longer required for the estates of individuals with a date of death on or after January 1, 2013. However, the Ohio Estate Tax is still in effect for individuals dying prior to January 1, 2013 who have a gross estate greater than $338,333.00.

As of July 1, 2009, pursuant to Superintendence Rule 45(D)(2), when personal identifiers are omitted from a case document submitted to the Court for filing, the party who submitted the case document shall submit the omitted information on a separate form. This form is not a public record and will not be maintained with the case file or imaged on the Court's website. This form (Personal Identifier Omission Form) is located in the court forms provided in this website.
