The Board of Miami County Commissioners' Human Resources team administers employee benefits for all offices except for the Health Department and Riverside. If you are an employee of either of those two offices please contact your manager for additional support. 

Forms, documents, and links included on the Human Resources pages are intended for use by employees of Miami County.

2024 1095-C forms were sent upon request, in accordance with the Paperwork Burden Reduction Act (H.R. 3797) update which became law on December 23, 2024.  

If you would like to request a copy of your 2024 1095-C form, you may do so using this link. You have the option to pick up the form from Human Resources or have it mailed to your home address.
